By submitting this form, you authorize NCTS Rideshare to conduct a criminal background check and motor vehicle record review as required under Wisconsin Statute § 440.48. You understand that you are responsible for the cost ($75.00) of this check, which will be reimbursed as a bonus after 90 days of active driving if you remain under contract.
